Procedure for Providing Academic Accommodations
Students with disabilities, special needs, or medical conditions are required to register with the Student Wellness Counseling by presenting necessary documentations in accordance with prescribed University guidelines. Consequently, students who present the accommodations letter to you, are instructed by the Dean's office to meet with you during your office hours to ensure their confidentiality and to discuss the accommodations outlined in the letter. To assist you in the implementation of the accommodation(s), the following details are highlighted:
After you read the accommodation letter, please sign it and keep a copy for your files. The student is responsible for returning the original copy to the Dean's office in order for the accommodation(s) to be implemented.
- If the accommodation is extended time for tests, then there are two available options. You should decide which option works for both you and the student.
- The first option is for you, the professor, to provide the time and space for the student to take the exam.
- The second option is for the student to take the exam in the Student Affairs testing facility. If this is the option that works for you and the student, then please coordinate with the student to make sure to notify Student Affairs staff at least seven days before an exam to make reservation for room and proctoring.
- If a student note-taker is needed, please make sure to solicit for a volunteer (without identifying the student who receives the accommodation) as soon as possible and refer the volunteer(s) to the Student Wellness Counseling. The Student Wellness Counseling will provide guidelines, answer questions, provides information regarding fee for note-taking service, and instruct the volunteer to drop notes for copying with the Student Affairs staff weekly. The student receiving accommodations is responsible for picking up the notes weekly from the Student Wellness Counseling. The note-taker does not need to know the name of the student(s) receiving this accommodation.
- If a tutor is needed, please make sure to solicit for a volunteer (without identifying the student who receives the accommodation) as as soon possible and refer the volunteer(s) to the Student Wellness Counseling. The Student Wellness Counseling will provide guidelines, answer questions, provide information regarding fee for tutoring service, get a signed confidentiality agreement, and make connection between the tutor and tutee.
- If there is an accommodation listed in the letter that causes concern, please do not hesitate to contact Student Wellness Counseling.
- Finally, if an issue arises during the semester regarding a student who has presented an accommodation letter to you, please consult with Student Wellness Counseling.
