Newly Hired Employees
New faculty and staff at Georgetown are required to submit a number of documents to HR in order to be set up in the HRIS/payroll system. HR will also need these documents in order to establish a NetID for the individual, which will enable the new employee to access email and certain areas of the website.

New Hire Documents for U.S. Taxpayers
Employees who are U.S. taxpayers must complete all of the following forms:- Employee Application
- Employee Supplemental Data Sheet
- I-9 Form and Instructions
- Direct Deposit Agreement or Local Payroll Deduction**
- Confidentiality Agreement
- W-4 Federal Tax Withholding
US Taxpayers must also complete one of the following tax forms, depending on their state of residence:
- VA-4 Virginia State Withholding Form
- MW 507 Maryland State Withholding Form
- D-4 District of Columbia Withholding Form
- D-4A Certificate of Non Residence in D.C.
Employees residing in Virginia, Maryland, or the District of Columbia should submit the corresponding withholding form. Employees residing elsewhere should use the D-4A Certificate of Non Residence in D.C. form.
Once completed, all forms should be submitted to David Phongsavan in the SFS-Qatar Human Resources Department.
New Hire Documents for Non-US Taxpayers
Employees joining SFS-Qatar who do not have a tax obligation to the United States must complete the following documents:
Employment Application
Employment Supplemental Data Sheet
Georgetown University Affiliated Non-Employee Transaction Form*
Confidentiality Agreement
Local Payroll Deduction **
W-8BEN Certificate of Foreign Status (Click here for an example of a completed form)
*The Affiliated Non-Employee Transaction Form is necessary to establish a record for the employee in our HRIS/Payroll system.
**In order to complete the Local Payroll Deduction Form, the employee must first have a local Qatar-based bank account. Please see Matt Tweito in the SFS-Qatar Human Resources Department for assistance in setting up a local bank account. This must be done after the employee has relocated to Doha. Employees with a U.S.-based banking account may opt to have all or part of their salary deposited into either the Qatari-based or US-based bank account.
Once completed, all forms should be submitted to David Phongsavan in the SFS-Qatar Human Resources Department.
