The following information must be turned in to Elizabeth Lopez:
- Student Employment Transaction Form
After being turned in to Elizabeth, this form will be signed by the student's Academic Dean (who will verify he or she is in good academic standing) and the Associate Director of Student Affairs (who will verify that he or she is not over-committed).
- Copy of passport information page
- Non-U.S. Citizens: ONE Tax Form
